Best HIPAA-Compliant Dental Practice Software

A ranking of HIPAA-compliant dental practice management options. We compare BAA terms, pricing models, and compliance fit for small dental clinics.

Decision summary

Dental practice management software is a mature category with strong clinical and imaging features. Compliance, on the other hand, is usually treated as a checkbox. This guide ranks five dental PM options plus PHIGuard, the flat-per-clinic compliance layer.

How we evaluated dental practice software

Dental software has a tighter shortlist than medical PM. Most US practices end up on Dentrix, Eaglesoft, Open Dental, or Curve Dental. We evaluated each on whether the vendor signs BAA details published on the pricing page, how pricing behaves as a practice adds operatories, and how the compliance program — not just the chart — is supported.

PHIGuard is in this list because dental practices have the same HIPAA obligations as medical practices, but compliance tooling for dental is even thinner. The PM holds the chart. Something else has to hold the BAAs, training, policies, and audit trail.

1. PHIGuard — compliance and task layer for dental practices

PHIGuard is the compliance and task management layer for dental practices with 3 to 50 staff. It is not a dental PM. It does not chart, schedule, or take radiographs. It handles the compliance program around your dental PM: BAAs with imaging vendors, labs, IT contractors, and any cloud service that touches PHI; written policies and procedures; annual training; incident response workflows; and an append-only audit log.

2. Dentrix

Dentrix from Henry Schein One is the most widely deployed dental PM in the US. It covers scheduling, clinical charting, perio charting, imaging integrations, billing, and insurance. Dentrix signs a BAA with covered entities.

Pricing is quote-based and varies by module and practice size. Dentrix is a solid operational fit; the compliance surface area outside Dentrix — every other vendor, every policy, every training record — still needs a system of record.

3. Eaglesoft

Eaglesoft from Patterson Dental is the other major server-based dental PM, with charting, imaging, scheduling, and billing. It is tightly integrated with Patterson’s hardware and imaging ecosystem.

Pricing is quote-based through Patterson. Like Dentrix, Eaglesoft is a competent PM for the operational side and a partial answer at best for the compliance side.

4. Open Dental

Open Dental is an open-source dental PM with a large installed base, particularly among independently-owned practices. It covers charting, scheduling, billing, imaging integrations, and patient communications. Pricing is published on the vendor’s site and is significantly lower per month than Dentrix or Eaglesoft.

Open Dental signs a BAA when applicable. Because it is open-source and self-hostable, the compliance burden — encryption, backups, access controls, audit logs — falls more heavily on the practice or its IT contractor.

5. Curve Dental

Curve Dental is a cloud-native dental PM that includes scheduling, charting, imaging, billing, and patient communications in a subscription. It signs a BAA with covered entities.

Pricing is subscription-based per office. Curve is a good fit for practices that want to avoid the server hardware and IT overhead of Dentrix or Eaglesoft.

Pricing models compared

Dental PM pricing splits into three buckets:

  • Quote-based, server-installed. Dentrix and Eaglesoft. Up-front investment plus ongoing support.
  • Published, server or cloud. Open Dental. Lower monthly cost, more practice-side responsibility.
  • Subscription, cloud-native. Curve Dental. Predictable per-month cost.

The compliance layer should not depend on which of those you choose. PHIGuard publishes current plan details on the pricing page.

Buying decision framework — 5 questions

  1. Does every vendor that touches PHI — PM, imaging, lab, IT, cloud backup — sign a BAA?
  2. How does pricing change when you add an operatory or hire a hygienist?
  3. Does your audit log capture image access, not just chart edits?
  4. Where do your written HIPAA policies and training records actually live?
  5. If a patient requests an accounting of disclosures, can you produce it from one system?

FAQ

Do I need PHIGuard if I already run Dentrix or Open Dental? Yes. The PM holds the chart. PHIGuard holds the compliance program around it.

Are dental labs business associates? If a lab receives PHI to fabricate a restoration, they are typically a business associate and need a BAA. PHIGuard tracks those agreements.
